Starting at $500 for the initial interview

This is a service for people who don’t have an immediate care management need but want to engage in service in advance of a healthcare crisis. A comprehensive assessment to include medical, psychosocial, etc. is completed. A follow-up is completed around the 6-month period. The follow up visits are billed at the care management rate. Any additional care management duties will be billed at the Care Management rate.
